The Hidden Risk of “Activated Charcoal” on McKinney’s Enamel
Activated charcoal toothpaste has an unusually convincing sales pitch.
It looks dramatic. It is marketed as “natural.” Your teeth may look cleaner after brushing. And compared with professional whitening, a tube or jar costs very little.
But there is a problem most social media demonstrations do not show:
Charcoal usually does not whiten teeth the way peroxide whitening does. Much of its effect comes from removing surface stains and depending on the formulation, that can involve abrasion.
Over time, repeatedly scrubbing away stains with an abrasive product can also contribute to wear of the tooth surface you are trying to make look better.
And enamel has one major limitation:
Once you physically wear enamel away, your body does not grow it back.
That does not mean every charcoal toothpaste will destroy your teeth. Research shows that abrasivity varies significantly between products, and some charcoal toothpastes have been tested within ranges seen in conventional toothpastes. But reviews have also found limited evidence that charcoal provides superior whitening and legitimate concern about the abrasive potential of some formulations.

Charcoal Does Not Really “Bleach” Your Teeth
There are two very different ways to make teeth look whiter.
One is to remove stains sitting on the outside of the tooth.
The other is to chemically change pigments within the tooth structure.
Charcoal mostly belongs in the first category.
Research suggests that charcoal products can remove or reduce extrinsic surface staining through their abrasive action and possibly by adsorbing some stain molecules. They generally do not significantly change the intrinsic color of the tooth the way peroxide-based bleaching can.
That distinction explains why someone can brush with charcoal and think:
“Wow. My teeth look brighter.”
They may.
But the product may primarily have polished away surface deposits rather than actually lightened the underlying shade of the teeth.
Professional bleaching and peroxide-based whitening work differently. Hydrogen peroxide and carbamide peroxide act on pigmented molecules within tooth structure rather than simply scrubbing the outside surface.
The Hidden Problem: Whiter Today Can Become Yellower Later
Here is the part that seems backward.
Enamel is not completely opaque.
The underlying layer of your tooth, called dentin, naturally ranges from whitish to yellow. The combination of enamel and dentin determines much of the color you see when you smile.
If enamel becomes progressively thinner, more of that underlying dentin can show through.
That can make a tooth appear more yellow, not less.
So excessive abrasion creates an ugly contradiction:

As enamel becomes thinner, patients may eventually notice increased sensitivity, discoloration, roughness, pitting, or greater vulnerability to dental problems.
That is why we would be cautious about using a highly abrasive whitening strategy every morning and night for years.
The concern is not usually what happens after three brushes.
It is what happens after thousands.
But Aren’t All Toothpastes Abrasive?
Yes.
And this is an important correction to some anti-charcoal claims.
Toothpaste needs some abrasive action to help remove plaque and surface stains.
Charcoal toothpaste is not automatically more abrasive than every conventional toothpaste.
One laboratory study evaluating 12 charcoal toothpastes found a very wide range of relative dentin abrasivity and relative enamel abrasivity. The researchers concluded that the overall ranges were not significantly different from ranges previously reported for conventional toothpastes. They also noted that abrasivity depends heavily on the amount, size, and shape of the abrasive particles, not simply whether the label says “charcoal.”
That is a useful context.
The problem is that consumers usually cannot look at black toothpaste and determine how abrasive it will be.
And other research has been less reassuring.
A systematic review found that most included studies identified higher abrasive potential with activated-charcoal formulations, while a 2024 laboratory study found charcoal dentifrices provided no better stain reduction than conventional toothpaste but produced greater long-term enamel wear in its test model. Loose activated-charcoal powder caused the greatest surface damage in that study.
So the responsible conclusion is not:
“Every charcoal toothpaste is dangerously abrasive.”
It is:
“The whitening benefit is weak enough that accepting uncertain abrasivity may not make much sense.”
Charcoal Powder Deserves Even More Caution
Loose charcoal powder can look more “pure” or “natural” than toothpaste.
That does not make it gentler.
In the 2024 laboratory study comparing several charcoal dentifrices with conventional toothpaste, the loose activated-charcoal powder produced the greatest changes in enamel roughness, gloss, and wear.
Powders also remove some of the safeguards you get from a formulated toothpaste.
A toothpaste manufacturer can control particle concentration, combine abrasives with other ingredients, include fluoride, and formulate a product for regular brushing.
Scooping loose charcoal powder onto a toothbrush gives you much less information about how abrasive the mixture is or how consistently you are using it.
If someone is determined to use a charcoal product, a formulated fluoride toothpaste is a more sensible conversation than brushing with loose powder.
What About Fluoride in Charcoal Toothpaste?
Older critiques of charcoal toothpaste often point out that many products lacked fluoride.
That remains worth checking, but the story has become more nuanced.
A 2025 study tested commercially available fluoridated charcoal toothpastes and found that fluoride remained chemically available and stable in the products examined. In other words, the presence of charcoal does not automatically mean the fluoride has been neutralized or rendered useless.
The practical rule is simpler:
Read the label.
If charcoal toothpaste is replacing your everyday toothpaste, make sure you are not also giving up proven cavity protection.

Who Should Be Especially Careful With Charcoal?
The concern becomes greater when a patient already has conditions that make the teeth more vulnerable to wear.
A review of charcoal dentifrices specifically recommended caution in people predisposed to dental wear, including those with gum recession, reduced saliva, high cavity risk, or existing dental erosion.
You should be particularly cautious if you already have exposed roots, sensitive teeth, visible enamel erosion, aggressive brushing habits, chronic acid reflux, frequent exposure to acidic drinks, or significant dry mouth.
Why?
Because abrasion does not happen in isolation.
For example, acidic exposure can temporarily make the tooth surface more vulnerable. Cleveland Clinic recommends waiting at least 30 minutes after eating before brushing when erosion is a concern rather than immediately applying additional mechanical abrasion.
If you drink coffee and immediately scrub your teeth aggressively with an abrasive whitening powder every morning, you may be solving one cosmetic problem in a way that creates another.
Charcoal vs. Actual Whitening
The difference becomes clearer when you compare what each method is trying to accomplish.

“I want my natural teeth several shades lighter,”
you are probably asking an abrasive toothpaste to perform a job it was never particularly good at.
What If You’ve Already Been Using Charcoal?
Do not panic.
Using charcoal toothpaste a few times does not mean you have permanently destroyed your enamel.
Tooth wear is generally a cumulative process.
If you have been using charcoal regularly and your teeth feel normal, the reasonable next step is simply to reconsider whether the product is giving you enough benefit to justify continuing it.
If you have developed new sensitivity, visible thinning, unusual yellowing, grooves near the gumline, rough surfaces, or changes in the shape of your teeth, have them examined.
Those symptoms do not prove charcoal caused the problem. Grinding, acidic foods and beverages, reflux, aggressive brushing, gum recession, and other conditions can produce similar changes.

Can You Repair Enamel After Charcoal Damage?
That depends on what “damage” means.

Actual physical enamel loss is different.
